Placement<\/strong><em>\u00a0(Florida State Statute 1003.21)<\/em><\/span><\/p><ul><li><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>Kindergarten:<\/strong>\u00a0A child must be five years old on or before September 1st.<\/span><\/p><\/li><li><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>First Grade:<\/strong>\u00a0A child must be six years old on or before September 1st\u00a0<strong>AND<\/strong>\u00a0satisfy one of the following: a) Satisfactory completion of kindergarten in a Florida public school. b) Satisfactory completion of kindergarten in a non-public school. c) Previous attendance in an out-of-state school in which the student was admitted on the basis of age requirements established by the state of residency.<\/span><\/p><\/li><\/ul><hr \/><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>Required Documentation Must be Uploaded With Online Enrollment:<\/strong><\/span><\/p><ol><li><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>Birth Certificate<\/strong><\/span><\/p><\/li><li><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>Court documents regarding custody, parental access and\/or name change<\/strong> (if applicable)<\/span><\/p><\/li><li><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>Physical\/Health Examination<\/strong>\u00a0(dated within 12 months of the student\u2019s enrollment date \u2013 can be from out of state) that meets the\u00a0<a href=\"http:\/\/www.floridahealth.gov\/programs-and-services\/childrens-health\/school-health\/_documents\/school-health-entry-exam-dh3040-chp-07-2013-instructions.pdf\">Florida Department of Health Requirements<\/a>.<\/span><\/p><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\">Sports physicals will not be accepted as enrollment physical\/health examination.\u00a0 Although a specific form is not required, you may choose to use the recommended\u00a0<a href=\"http:\/\/www.floridahealth.gov\/programs-and-services\/childrens-health\/school-health\/_documents\/school-health-entry-exam-form-dh3040-chp-07-2013.pdf\">School Entry Health Exam Form (DH3040)<\/a><\/span><\/p><\/li><li><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>Florida Certification of Immunization, DOH 680 Form<\/strong><strong>\u00a0<\/strong><em>(Florida State Statute 1003.22)<\/em><\/span><\/p><\/li><li><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>Proof of Residency \u2013 All the following documents are required for a complete proof of residency packet:<\/strong><\/span><\/p><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\">a. Current mortgage statement, or copy of Deed or current lease. Purchase and sales contracts are not acceptable as proof of\u00a0 residency; AND<\/span><\/p><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\">b. One utility bill (gas, water, electric or internet) dated within 30 days. This may also be activation notice (Certificate of Service) from utility company showing name, address and start date of service; AND<\/span><\/p><p><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\">c. Second bill or statement, different from one provided above, dated within 30 days, showing name, address and date.
