Registration Information

REGISTRATION INFORMATION FOR NEW SEA TURTLES
 
If you wish to enroll your student for the remainder of the 2024-2025 school year, you must complete the appropriate enrollment form and UPLOAD ALL REQUIRED DOCUMENTATION WITH THE FORM.  You will not be permitted to submit without uploading all required documentation.  See below for list of required documentation.

Once you have submitted your NEW STUDENT registration form, Ms. Jason will reach out to you to confirm your child’s registration and next steps in the process.

REGISTRATION REQUIREMENTS

Requirements for Children to Begin School: Grade Placement (Florida State Statute 1003.21)

  • Kindergarten: A child must be five years old on or before September 1st.

  • First Grade: A child must be six years old on or before September 1st AND satisfy one of the following: a) Satisfactory completion of kindergarten in a Florida public school. b) Satisfactory completion of kindergarten in a non-public school. c) Previous attendance in an out-of-state school in which the student was admitted on the basis of age requirements established by the state of residency.


Required Documentation Must be Uploaded With Online Enrollment:

  1. Birth Certificate

  2. Court documents regarding custody, parental access and/or name change (if applicable)

  3. Physical/Health Examination (dated within 12 months of the student’s enrollment date – can be from out of state) that meets the Florida Department of Health Requirements.

    Sports physicals will not be accepted as enrollment physical/health examination.  Although a specific form is not required, you may choose to use the recommended School Entry Health Exam Form (DH3040)

  4. Florida Certification of Immunization, DOH 680 Form (Florida State Statute 1003.22)

  5. Proof of Residency – All the following documents are required for a complete proof of residency packet:

    a. Current mortgage statement, or copy of Deed or current lease. Purchase and sales contracts are not acceptable as proof of  residency; AND

    b. One utility bill (gas, water, electric or internet) dated within 30 days. This may also be activation notice (Certificate of Service) from utility company showing name, address and start date of service; AND

    c. Second bill or statement, different from one provided above, dated within 30 days, showing name, address and date. This can be a bank statement, cell phone bill, credit card bill or driver’s license IF driver’s license has residency address.

  1. Valid Driver’s License/ID card for guardians (for identification purposes).

  2. ESE IEP/EP/504 plan/RTI documentation (if applicable)

  3. Latest report card from prior school.

SCHOOL ACCESS

In a continuing effort to provide safety and security to our students and staff we ask that you complete a School Access Form if you plan to enter a facility in the St. Johns County School District. Any person passing beyond the front office area must show a valid driver’s license or state identification card: School Access Form

CHANGE OF ADDRESS

For a change of address, please use the link to the right “Change of Address/Transfer Form”.  To process your change of address application, you must upload all required proof of residency documentation per the Residency and Guardianship Policy.
